Practice Policies

Greetings! All patients are required to read and adhere to these practice policies.

Invoices/Balance

Due to multiple issues with collecting payments it has become necessary to form the below policies.

All patients must pay their balance prior to their scheduled appointment. If a patient fails to pay an invoice, they will not be able to have virtual appointments. The patient must come in-person for all of their future appointments.

If you are a patient of Dr. Keise’s and fail to pay your invoice prior to your appointment, you will have to see one of our other providers in-person for your future appointments.

If the patient is unable to come in-person, they will have to deposit $400 in cash, which will cover copays, deductibles, and no-shows. This must be replenished when it decreases below $200.

Payments

For payments, you can pay with a credit card or zelle

For credit card  payments you may pay through the invoice sent to you.

For Zelle payments send via Zelle to drkeise@insytepsychiatric.com.

Insurance Coverage and Deductibles

All patients are responsible for knowing their insurance coverage. Patients who have a deductible will be charged 48-hours prior to their appointment. We are currently in network with Blue Cross Blue Shield, Aetna, and Cigna plans with the exception of any plans through Medicaid or Medicare. All patients are responsible to pay any claims rejected by their insurance.

Insurance and Information Changes

Any changes with your insurance, phone number, residence address, and email address should immediately be reported so we can update them in your chart. If a claim is rejected due to failure to update information, the patient is responsible for the full fee of the visit and it will not be our responsibility to resubmit the claim.

Insurance changes MUST be reported 48 hours prior to your appointment, otherwise you will be billed for the appointment.

Required Documents

Required documents we need to have on file for the patients include:

·   Valid photo ID

·   Insurance Card: front and back

·   A valid credit card should be saved on file along with a signed credit card authorization form

Cancellations/Rescheduling

To cancel or reschedule your appointment please email info@insytepsychiatric.com no later than 48 hours prior to your scheduled time. You can also call the office at 732-515-5089. If you are unable to reach us, you must leave a voicemail requesting the appointment be cancelled.

Missed appointments or cancellations without 48 hours of notice will result in a $200 (non MD) $400 (MD) no-show fee for follow up appointments, and a $400 (non MD)/$800 (MD) no show fee for initial evaluations.

If you are late to your appointment by 15 minutes or more, it is considered a no show and you will be charged the no show fee.

Patient Treatment

Here at Insyte Psychiatric, all of the providers work closely together to provide patients with great care. Dr. Keise works with PA Kimberly McCullough and Psychiatric nurse practitioner Ruth Yamoah-Cofie. All of our providers are certified to prescribe medication and implement treatment plans.

This is an Adult practice, providing services to patients who are 18 years and older. Care will be provided through the patient only not via spouse or parent. If a patient wishes to have another party present with them at session, they must first sign a release form on our website naming the party present in the session. No information will be given to a 3rd party ( i.e spouse , parent or friend) outside of an appointment without the patient present.  Treatment will ONLY be discussed with the patient during their session. If there is a release form for a family member, including parent or partner, the patient must be present to discuss any matter. We will not be responding to any emails or calls on behalf of the patient, this includes scheduling or canceling appointments made by a 3rd party. All patients must give their own phone number and email address on file. If you need to update this information, please reach out to us via email or phone call.

Telehealth Appointments

Patients are also required to be on video for their telehealth appointments in order for their session to be covered by their insurance. If the patient is unable to be on video for their session, they will have to pay out of pocket. Patients cannot be driving during their telehealth appointments, otherwise the session will be marked as a no show.

Post IOP Appointments

If patients are creating an appointment after completing an inpatient program, they will be required to provide us with their discharge medication summary prior to their appointment. This can be done by reaching out to the facility and requesting them to send us the summary via email or fax.

Urine Test for Controlled Substances

Patients that are on a controlled substance will be given a script for a urine test once every 3 months. They will be required to submit the results within 72 hours to get a refill on their medication. Patients that are unable to submit this will not be able to get refills.

Medication Discussion

Any discussion regarding medication should be addressed in the session with your provider. If you are experiencing any side effects or have questions about your medication after your session, you will be scheduled for an appointment with your provider to discuss them. This is to ensure that no errors are made in treatment and that questions are addressed thoroughly. In case of an emergency session if your provider does not have availability you may be scheduled with another provider in the practice for that appointment to ensure that your issue is addressed in a timely fashion.

If you would like your provider at Insyte Psychiatric to discuss anything with another healthcare provider or facility, a release form is required with the name and contact information of the healthcare provider/facility. The form can be found on our website under “Patient Release Form.” The outside provider should then make an appointment with your provider.

Medical Records

If you would like us to send your medical records to another provider or facility, please sign the release form on our website. Please note, this may take up to two weeks to be processed.

If you would like to pick up printed copies of your medical records, there is a charge for $1 per page up to $100. The patient must pick up their own records.

Forms and Letters

Any forms and letters you may require from your providers first need to be discussed with your providers during your session. Some paperwork requires that we have a relationship with the patient for longer than six months to a year in order to appropriately complete.

Once they are approved by the provider, they may take up to two weeks to be completed. There is a $25 charge per page for all paperwork.
